Wellbeing Planner for an Older Parent
A simple weekly planner for families supporting an older relative. Change in later life happens in small steps that each look reasonable on their own, so families usually notice a decline late. Writing a few things down each week makes the trend visible instead of leaving it to memory, stops several relatives all assuming somebody else visited, and gives a GP, social prescriber or needs assessor something concrete to work from.
What to track: visits received and roughly how long they stayed, general mood scored from one to five based on the conversation, appetite and whether meals are being cooked or skipped, sleep including daytime napping, and activity such as whether they left the house at all. A notes column covers falls, appointments and good days. Review it with a glance each week and a proper look once a month across four sheets, watching for visits dropping, mood drifting down, or three quiet weeks in a row.
